If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

Emergency / After hours information:

In case of an emergency, I can be reached by telephone at (801)944-8700, which will be forward to my cell phone. If you are unable to contact me and need immediate medical attention, please call 911 or go to your nearest ER.
